Student Medical Forms

Medication orders EXPIRE on June 30th of the current school year.  NEW medication orders must be dated July 1st or later , and must be submitted to the school nurse no later than the first day of school in order for your child to attend.  Failure to provide medical documentation could result in exclusion from school if medically appropriate.
